Upon receipt of this completed registration form and your payment either by check or school purchase order, your space will be secured and an email confirmation will be sent.

Early registration is strongly recommended as space is limited. Registration and check or PO must be received by or before the early registration date noted to qualify for the discount .

Cancellations made less than 7 days before the event will not be eligible for refund.

Weather or other unforeseen occurances may result in a need for rescheduling or cancellation.  In the event of a cancellation a full refund will be awarded.  LTR is not responsible for any personal or professional fees or charges that may result due to  rescheduling or a cancellation. Please know that LTR makes every effort to avoid these changes.
